OUR PROGRAMS

About our programs

The North Simcoe Family Health Team is comprised of both primary care providers and allied health providers (educators, social workers, dietitians, etc.) that are here to help you and your family become the healthiest you! We do this by providing a range of programs that focus on health promotion, disease prevention and self-management skills. Our team believes that by providing a person-centred and holistic approach to care, each individual will have the support and tools to guide their health care journey. Our programs are aimed at meeting the needs of our community and are open to anyone residing in the North Simcoe region. *Please note: some of our programs require a doctor or nurse practitioner referral.
