SENIORS CARE EXERCISE PROGRAM
About
The Seniors CARE Exercise Program is a group program targeting frail seniors that focuses on improving mobility, strength, coordination, and balance. Each class includes physical exercises, brain games, and an education component covering a variety of topics. This program is delivered by Registered Kinesiologists.

Location & Hours
This service is located at the North Simcoe Family Health Team's main office at 619 Prospect Blvd., Suite 3, in Midland. This program runs for 12 weeks. Classes run two times per week for two hours. Days and times vary.

How to Register
This program is open to anyone in the North Simcoe Community. There are two ways to register:

To self-refer, click HERE and fill out the NSM Referral form. Please fax completed referral to SGS Central Intake at 705-792-4614.

Obtain a referral from your Physician, Occupational Therapist or Nurse Practitioner (they can click HERE to refer).
