DIABETES EDUCATION

About

Our Diabetes Education Program will support you in living a healthy lifestyle with improved diabetes management. Your care team will consist of registered Nurses, dietitians, social workers, and advanced foot care nurses.

This program is accessible to individuals who:

  • Are at risk of diabetes, have Type 1, Type 2 or gestational diabetes

  • 18 years or older

  • Want to make informed decisions about their diabetes management to reduce long term complications

Topics Covered at Appointments

During your appointment the following may be reviewed:

  • Have your blood pressure checked

  • Teach you how to check your blood sugar to see patterns

  • Talk about healthy eating and exercise

  • Discuss how your medication and insulin work

  • Have a foot assessment completed

  • Learn how to set goals

  • Review your lab tests (A1c, cholesterol, kidney function)

  • Discuss your feelings and emotions about having diabetes

Location & Hours

This program is located at the North Simcoe Family Health Team's main office at 619 Prospect Blvd., Suite 3, in Midland. Appointments occur Monday to Friday.

How to Register

This program is open to anyone in the North Simcoe Community. You may access this service by either a physician referral or self-referral.
