About

This service was developed to help patients experiencing hypertension. Patients that come to this program may already be diagnosed with high blood pressure or concerned that they may have hypertension. This program may be used to rule out white coat syndrome, monitor blood pressure or to access 24hr ambulatory blood pressure monitoring. Heart Health follows "Evidenced Based Best Practice Guidelines for Hypertension Management."

Your First Appointment

During your first appointment, we will cover:

  • What hypertension and blood pressure is

  • Home and ambulatory blood pressure monitoring

  • Blood Pressure monitoring for newly diagnosed patients, stabilized patients, patients needing reassessment, and ruling out white coat syndrome

  • Lifestyle and self-management education

  • Review of anti-hypertensive medications

Please bring with you:

  • A list of your current medications including over the counter medications

  • Blood pressure readings and your home blood pressure monitor if applicable

  • Any questions you may have about your blood pressure

Location & Hours

This service is located at the North Simcoe Family Health Team's main office at 619 Prospect Blvd., Suite 3, in Midland. Appointments occur Monday to Friday.

How to Register

This service is open to anyone in the North Simcoe Community. You may access this service by either a physician referral or self-referral.
