About our services

The North Simcoe Family Health Team is comprised of both primary care providers and allied health providers (educators, social workers, dietitians, etc.) that are here to help you and your family become the healthiest you! We do this by providing a range of services that focus on health promotion, disease prevention and self-management skills. Our team believes that by providing a person-centred and holistic approach to care, each individual will have the support and tools to guide their health care journey. Our services are aimed at meeting the needs of our community and are open to anyone residing in the North Simcoe region. *Please note: some of our services require a doctor or nurse practitioner referral.

After Hours Clinic

The After Hours Clinic provides extended appointment hours to patients who belong to a Family Health Network (FHN) physician only. This clinic is not open to the public.

Cardiovascular Rehab

This service is focused on improving the detection and self-management of cardiovascular health problems (e.g. Acute Coronary Syndrome, Bypass Grafts, Percutaneous Coronary Intervention).

Diabetes Education

Our Diabetes Education Program will support you in living a healthy lifestyle with improved diabetes management. Your care team will consist of Registered Nurses, Dietitians, and Social Workers, Advanced Foot Care Nurse and a Nurse Practitioner.

Dietitian Services

A dietitian will provide a 1 hour initial assessment and 30 minute follow-up appointments as needed. We also offer a series of dietitian-led programs throughout the year.

Heart Health

This service was developed to help patients experiencing hypertension. Patients that come to this program may already be diagnosed with high blood pressure or would like to know if they do have high blood pressure.

Injection Clinic

This clinic provides injections to individuals to help ensure you receive your regular injections in a timely manner.  Injections are administered by a nurse.

Lung Health

This service is focused on the self-management of respiratory health problems. Patients make appointments with a Certified Respiratory Educator to build confidence in managing their condition.

Mental Health

Our Mental Health team provides brief, strengths-based and solution-focused counselling interventions. We provide services to children, youth and adults.

Same Day/Next Day Clinic

Our Same Day/Next Day Access Clinic is available to all residents in the North Simcoe community. This is not a walk-in clinic. All appointments are with a Nurse Practitioner.

Telemedicine

This service is provided through the Ontario Telemedicine Network (OTN).  OTN is a secured network that uses two-way videoconferencing systems to connect you with health care members and specialists across Ontario.

Seniors Care

This service provides frail seniors with assessments and on-going support from an interdisciplinary team to help you stay safe and healthy in the community that you live!

Well Baby Clinic

This clinic provides care for infants from birth to 2 years of age. The focus of this clinic is to monitor your infant’s growth and development, collaborate with your primary care provider and provide referrals to community programs and early intervention treatment services.

Well Child Clinic

The Well Child Clinic provides care for children at 2 years of age and again prior to starting kindergarten. This clinic assesses your child’s growth and development, nutrition and activity levels.
