FALL PREVENTION PROGRAM

About

The goal of our Fall Prevention Program is to help seniors maintain independence and dignity through proactive fall prevention. The NSFHT Fall Prevention Program helps community dwelling adults (aged 65+) reduce their risk of falls and maintain their independence through a comprehensive fall risk assessment, balance and mobility screening, and personalized goal setting. Participants will receive education and practical strategies to stay safe and active at home and in the community. 

Individual Balance/Falls Assessment

During the appointment our staff will review with the patient:  

  • Fall history review 

  • Identification of fall risk factors 

  • Functional review (activities of daily living, instrumental activities of daily living) 

  • Orthostatic blood pressure screening 

  • Medical review 

  • Medication review 

  • Balance and mobility assessment 

  

Our staff will provide participants with education on fall prevention, personalized goal setting, and referrals to community exercise programs or other wellness programs and supports (when appropriate). 

Group Fall Prevention Education

A group education session is offered quarterly to learn general information on balance and fall prevention. Topics covered during the education session include; 

  • Managing chronic health conditions for fall prevention 

  • Ways of reducing fall risks in the home environment 

  • How medications can impact balance  

  • Local Services and programs that can be accessed to support individuals 

Location & Hours

This service is located at the North Simcoe Family Health Team's office at 619 Prospect Blvd., Suite 3, in Midland. Appointments occur Monday to Friday.
