COVID-19 VACCINATION

About

Our office is currently offering all doses of the COVID-19 vaccinations. Eligible groups for 2nd booster doses (4th dose) may book at least 5 months or 140 days from when they received their 1st booster dose.

 

Eligible groups include:

​

  • Individuals ages 18 and over.

  • Health care workers and designated essential caregivers in congregate settings who are 16+.

  • Indigenous people and their non-Indigenous household members who are 16+.

  • Individuals who received a complete series of viral vector vaccine (two doses of AstraZeneca/COVISHIELD or one dose of Janssen vaccine).

At Your Appointment

Please keep the following information in mind at your COVID-19 vaccination appointment:

  • Arrive on time, NOT any earlier

  • You will be screened for COVID-19 symptoms upon arrival

  • Wear your mask and sanitize your hands

  • Verbally consent to the vaccination

  • Wait 15 minutes after getting your vaccine before leaving

Location & Hours

This service is located at the North Simcoe Family Health Team's main office at 619 Prospect Blvd., Suite 2, in Midland. Appointments occur Monday to Friday from 8:15 - 4:00pm.

How to Register

This service is open to anyone in the North Simcoe Community that is 18+ years of age. You may access this service by self-referral. To book an appointment, please call 705-526-7804 ext 0.
